We ask that all potential new chiropractic adjustment practice members fill out the new patient paperwork forms as completely as possible. If you are unable to print out this paperwork, please arrive 15 minutes prior to your appointment at our Brookfield chiropractic office. It is also important to bring in a form of ID as well as any insurance cards that you would like to have a complementary benefits check run on.
Next, you will sit-down for a one-on-one consultation with one of our chiropractic doctors. This is where you will be free to address any health concerns and goals that you may have, as well as addressing any past damages and traumas you may have endured that will help us address the cause of your health concerns.

A comprehensive chiropractic evaluation will be performed to further determine the exact cause of your health problems. The doctor will perform necessary orthopedic, neurological, and physical exams based on your presentation and health history.
The Doctor will advise you if there are any necessary further examinations or X-Rays that will need to be taken based on initial exam findings. State of the art digital x-rays are performed on site.

Once the doctor has completed their review of your assessment and examination, a customized plan of care will be created specifically for your individual needs. At this time, the doctor will present a detailed report of their findings. We then discuss your customized plan of corrective chiropractic care and payment options moving forward.
